ALL STUDENTS - If you do not need the Medication Authorization form, you need to mark it N/A and still submit it.

If your physical is scheduled for after July 27, you must start your Aktivate account and fill out the paperwork you can.

An EKG/ECG is not required to participate in band.

All GCHS Shark Band students must register on Aktivate to participate in band.

A current physical is required to be downloaded to aktivate.

Band students DO NOT need to complete the EKG/ECG requirement. Only students who participate in band and another school sport must complete this additional form.

The 2026-27 school year is now open in Aktivate.
